The primary care department at Bartz-Altadonna Community Health Center offers preventive care and treatment for our patients, every new patient that comes to our clinic must first be screened by our primary care providers, so this department is the first point of contact to be linked to other specialty services.
In this department new patients can receive medical treatment, nutritional services, and education for several chronic conditions like Diabetes, Chronic Obstructive Pulmonary Disease (COPD), Asthma, Hypertension, etc., and referrals to any specialty services that patients may need.
Bartz-Altadonna Community Health Center maintains relationships with many specialty providers and is able to link new patients to those services.
We offer reproductive health services, family planning services including counseling and birth control. As a Fampact provider we are able to provide services to patients who are uninsured.
We offer a full range of services for Women’s health including cervical cancer screenings, referrals to mammograms, and we can also help uninsured patients enroll into the program Every Woman Counts, which offers free cervical and breast cancer screening and diagnostic services to California residents.
We offer free Vaccines for adults through the Vaccines for Adults program (VFA) for eligible patients.
“FREE vaccines are available to adult patients who are uninsured or have insurance that doesn’t cover (certain) vaccines. We do not charge these patients for getting the vaccine or for the cost of the vaccine.”
